Medical and Dental Examinations

The State of Illinois requires that students entering pre-school programs, kindergarten or entering a public school for the first time, sixth grade, and ninth grade must turn in a completed physical examination. A lead screening is also required for all students entering pre-school or kindergarten. Kindergarten students must also complete and submit an immunization booster series.

The State of Illinois has mandated that all students entering kindergarten or enrolling for the first time in an Illinois school are required to submit proof of an eye exam performed by a qualified eye doctor. All proof of eye examinations must be presented to the school by October 15th. If a child in kindergarten or entering an Illinois school for the first time fails to present proof of the required eye examination, the school may hold the child's report card until: child presents proof of eye examination form or child presents an eye examination waiver form.

Any physical, immunization update and lead screening should be done before school actually starts. Students who have not completed these requirements may be excluded from school within 60 days of Registration/Fee Day or by October 15th, whichever is earlier, until they are turned in.

All Illinois children in Kindergarten, second and sixth grades are required to have a dental examination. This examination must be performed by a licensed dentist and he/she will sign a proof of examination form. Each child shall present proof of examination by a dentist prior to May 15 of the school year. If a child in second or sixth grade fails to present proof by May 15th, the school may hold the child's report card until: child presents proof of dental examination form or child presents a dental examination waiver form.
